Re: Official Warning

This letter serves as an official warning for failure to submit your
data collection forms. On August 2nd, 11th, 22nd, and November 6th, I
sent you reminder emails about your data collections being late. In
addition, I have given you several verbal reminders at the YiA
AmeriCorps office. After your last verbal reminder you assured me
that you would correct this problem. Yet my records show that you
have not emailed me the following:

June data collection, Due 07/07/2005
July data collection, Due 08/04/2005
August data collection, Due 09/08/2005
September data collection, Due 10/06/2005
October data collection, Due 11/03/2005

It's important that you turn in your data collection forms. First,
your reporting allows our AmeriCorps program to demonstrate how it is
impacting the community; which allows us to apply for grants to
sustain the program. Secondly, we are required to submit your data
and progress to the Governor's office. Your failure to report data
collections also requires me to spend extra time giving you multiple
reminders, verbally, via the phone, through email. When you do not
turn in data collections it causes a disruption in the operation of
our program.

You have one data collection due each month. You are expected to
submit your data collections in a timely manner according to the
established schedule. Data collection due dates can be found at our
website (www.nau.edu/americorps), via listserve email reminders,
newsletter reminders, and by viewing the member calendar by the
mailbox area. In order to assess your progress in resolving this
issue, your late data collection forms will be completed and submitted
to my email address, {OMITTED} by Friday, December 9th by
5pm.

Failure to turn in your data collection reports by May 6th will result
in disciplinary action.

A copy of this letter will be placed in your AmeriCorps file and
another copy will be sent to your Mentor, Chris Baker.
